To import seizure data, your CSV file must use the following column order: 

    Date, Time, Duration, Rescue Medication, Type, Trigger, Activity, Location, VNS, Note, Severity, Mood       

       

  - Date — yyyy-MM-dd (e.g. 2026-01-15)                                                                                                                                 

  - Time — 24h format HH:mm (e.g. 14:30)                                                                                                                            

  - Duration — mm:ss (e.g. 2:30)                                                                                                                                                

  - Rescue Medication — 0 for none, or the number of times given (e.g. 2)                                                                

  - Severity / Mood — integer from 1–5, or -1 if not recorded           

                                                                                   

All text fields (Type, Trigger, Note, etc.) must be wrapped in double quotes. Download the template below for a ready-to-fill example.
